The following terms and conditions constitute a legal and binding contract for the adoption of the dog listed. These terms and conditions are entered into and agreed upon by both Compact Canine Rescue and the adopter and they acknowledge the legal worth of this contract. To the best of Compact Canine Rescue's knowledge, this dog has no undisclosed physical defects or issues that would make it unsuitable as a family pet. Adopter agrees to all of the following terms of this contract and understands that non-compliance with the terms of this agreement gives Compact Canine Rescue or their representatives the right to reclaim this dog without refund of adoption fee.

The dog is being transferred to the adopting owner with the understanding that the adopter is taking possession of the dog to treat and to be responsible for it as their own dog.
The dog will be treated as a family member with loving care and affection. I will make every reasonable effort to ensure the dog’s safety and well-being. The dog will live inside my home and will not be isolated from the family.
I will feed the dog at least twice a day and will provide a fresh supply of water at all times.
Within 24 hours of adoption, I will place identification on the dog with tags and a collar. Identification includes the following: the dog’s name, owner's name and phone number, at a minimum. Identification must be readable at all times.
I will ensure proper licensing of the dog and will attach the appropriate license tags, rabies tag, and personal identification tag to a non-choke collar to be worn at all times. I will ensure compliance with all applicable local and state statutes.
The dog will be walked on a leash or be exercised in a fenced yard on a daily basis. I will never allow my dog run loose or roam, keep my dog chained or tied up, keep it continuously in a yard, garage, patio, balcony, or pen, or leave my dog outdoors, even in a fenced yard, unsupervised or when no one is at home.
I will take the dog to my licensed veterinarian within 30 days of adoption to establish a new patient record and for a wellness visit. I will provide annual veterinary examinations, monthly heartworm preventative, yearly vaccinations, rabies according to my state’s regulations, monthly flea and tick prevention medication and dental care as determined appropriate by my dog’s veterinarian.
I understand and acknowledge that Compact Canine Rescue cannot make any guarantees, either expressed or implied, concerning the health, habits, temperament, background or age of the dog I am adopting from them. I understand that all dogs are heartworm negative when adopted out, but that I must re-test for heart worms six months after adoption. I understand and acknowledge negative tests are not always a guarantee that the dog is heartworm free.
In most cases the adopted dog will have been spayed or neutered. If not already altered, I will have the dog spayed/neutered within 90 days of adoption and will immediately forward proof to the Compact Canine Rescue office. Failure to comply with this requirement will result in the immediate return of the dog to Compact Canine Rescue. Spay/neuter amendment is attached, if appropriate. The following statement is required by Virginia law: STERILIZATION OF ANY ADOPTED ANIMAL IS REQUIRED BY TITLE 3.2, CHAPTER 65, SECTION 3.2-6575 OF THE CODE OF VIRGINIA. THE ADOPTER AGREES TO ASSUME FULL RESPONSIBILITY FOR COMPLIANCE WITH THE PROVISIONS OF 3.2-6574, WHICH PERTAIN TO THE TIMELY STERILIZATION OF THE ADOPTED PET. A PERSON WHO VIOLATES THIS ARTICLE IS SUBJECT TO CIVIL PENALTY. THE NEW OWNER MAY BE COMPELLED TO COMPLY WITH THE PROVISIONS OF THIS ARTICLE.
I agree to follow positive training methods. Use of prong collars, choke collars or shock collars for any reason is expressly prohibited.
I am adopting the dog for myself and I agree to not give away, sell, or trade the dog, even as a gift to a friend or family member. I will neither take the dog to a shelter nor abandon the dog. I understand that I must notify Compact Canine Rescue, without delay, if I can no longer care for or keep the dog and agree to give Compact Canine Rescue reasonable time to rehome the dog or place the dog in an approved foster home, if available. I must notify Compact Canine Rescue of any behavioral problems that have occurred at any time before I return the dog and I agree to pay for a professional trainer’s evaluation in any case of biting or aggression. I agree the dog will not be euthanized in a non-emergency situation without discussing with Compact Canine Rescue. Should the dog be returned to Compact Canine Rescue, I understand there is no refund of all or any part of the adoption fee.
I agree to notify Compact Canine Rescue within four (4) hours if this dog is lost or stolen and to make every effort to locate the dog. Notification to Compact Canine Rescue at 804-721-4703 and email to Sharon@compactcaninerescue.org and Susannah@compactcaninerescue.org.
I agree to accept responsibility and ownership of the dog at my own risk and I release Compact Canine Rescue and its agents from any and all liability arising out of possession and ownership of my dog. I agree that I am assuming total financial responsibility for the dog as of the date of this contract. Compact Canine Rescue and its agents will not be held responsible for any damages or expenses (veterinary or other) incurred during my ownership of the dog.
I agree to notify Compact Canine Rescue if I relocate(s) from my current residence and provide the new email address, physical address and phone number. Notification to Susannah@compactcaninerescue.org.
I affirm that no member of my household has been convicted of an animal welfare law violation such as neglect, cruelty, abandonment, etc.
This dog’s known background and medical history have been discussed with me. I understand that Compact Canine Rescue has made no representation concerning the health, condition, training, behavior or temperament of the dog.
I agree to permit Compact Canine Rescue to make inquiry about and enforce any of the above conditions and requirements at any time after adoption. This can include visits to my home and contact with my veterinarian. I UNDERSTAND THAT FAILURE TO COMPLY WITH ANY OF THE ABOVE PROVISIONS WILL RESULT IN FORFEITURE OF THE DOG TO COMPACT CANINE RESCUE.
ENFORCEMENT OF CONTRACT I understand that by voluntarily signing this agreement, I am entering into a legal and binding contract with Compact Canine Rescue. Breach of any term(s) of this agreement is deemed actionable by Compact Canine Rescue. In the event there is a violation of the agreement, I agree to pay a minimum of $500.00 in damages. Additionally, in order to facilitate the collection of damages for breach of contract, I waive any challenge to venue and agree that the appropriate venue for this matter is the Commonwealth of Virginia, and the County of Norfolk. Furthermore, I agree to accept service of process by certified mail, return receipt requested, to the address provided in this adoption contract, and specifically waive any right to receive personal service. All such causes of action filed in Norfolk County, Virginia shall be submitted for binding arbitration upon written request of either party. The parties shall have 10 calendar days from the date the written notice is sent to the opposing party at the opposing party's last known address to agree on an arbitrator. If the parties are unable to agree on an arbitrator, Compact Canine Rescue shall select the arbitrator. If a party fails to appear for the noticed arbitration, that party shall be considered in default and all relief requested by other party shall be granted in full.
RELEASE OF LIABILITY AND INDEMNIFICATION Adopter promises and agrees to be solely responsible for this animal and to indemnify and hold harmless adopting agent from any and all claims of liability for the conduct of this dog on or after the date of this adoption and agreement of contract. The undersigned adopter has been advised of these facts and agrees to all conditions of this contract.
The facts stated in my submitted Adoption Application are true.
